Pursuant to the Orders of the Ministry of Health Sverdlovsk region and Office Federal Service on supervision in the field of consumer protection and human well-being in the Sverdlovsk region dated 04.20.2011 N 383-p / 01-01-01-01 / 188 "On amendments to the Order of the Minister of Health of the Sverdlovsk region and the head of the Office of Rospotrebnadzor in the Sverdlovsk region dated 21.06 .2003 N 434-P / 01 / 1-176 "On measures to improve the organization of occupational pathological care in the Sverdlovsk region" and dated 11.04.2012 N 360-p / 01-01-01-01 / 127 "On improving the system of organizing and conducting mandatory preliminary and periodic medical examinations (examinations) of employees of the Sverdlovsk region", in order to timely sum up the results of the work of healthcare institutions in the Sverdlovsk region for 2014

I order:

1. The chief physicians of the state budgetary healthcare institutions of the Sverdlovsk region must submit reports on occupational pathology for 2014 (Appendices No. 1, 2, 3) to the federal state-financed organization Science "Yekaterinburg Medical Research Center for Prevention and Health Protection of Industrial Workers" of the Federal Service for Supervision of Consumer Rights Protection and Welfare, Head of the Department of Organization of Occupational Health Gazimova V.G. (at the address: Ekaterinburg, Popova st., 30; room N 215, 217; tel. 359-84-86).

2. Recommend to the head of the Yekaterinburg City Administration Dornbush A.A. ensure the submission of reports on occupational pathology municipal institutions health care of Yekaterinburg in accordance with annexes N 1, 2, 3.

3. To the chief freelance specialist-occupational pathologist of the Ministry of Health of the Sverdlovsk Region N.O. Milovankina:

1) monitor compliance with the deadlines for submitting reports;

2) before 03/01/2015 to analyze the results of the work of medical institutions on the organization of occupational care for the population of the Sverdlovsk region and preventive examinations of workers in harmful conditions labor;

3) submit an analysis of the work of the occupational pathology service of the region to the Ministry of Health of the Sverdlovsk region.

4. To impose control over the execution of this Order on the Deputy Minister of Health of the Sverdlovsk Region E.A. Chadova.

ON THE APPLICATION OF THE LIST OF CERTAIN TYPES OF PROFESSIONAL

ACTIVITIES AND ACTIVITIES RELATED TO THE SOURCE

OF INCREASED DANGER FOR WHICH ARE ESTABLISHED

RESTRICTIONS FOR PATIENTS WITH ADDICTION, APPROVED

DECREE OF THE GOVERNMENT OF THE RUSSIAN FEDERATION

1. Article 45 of the Federal Law of January 8, 1998 N 3-FZ "On Narcotic Drugs and Psychotropic Substances" (Collected Legislation of the Russian Federation, 1998, N 2, Article 219) establishes that in order to protect health, morality, rights and the legitimate interests of citizens in the Russian Federation, restrictions have been established on engaging in certain types of professional activities and activities associated with a source of increased danger for drug addicts.

In accordance with this article, Decree of the Government of the Russian Federation of May 18, 2011 N 394 (hereinafter referred to as the Decree) was adopted, which approved a list of certain types of professional activities and activities associated with a source of increased danger, for which restrictions are established for drug addicts (hereinafter - list).

2. When a person enters a job for those types of activities that are indicated in the list, as well as in the process labor activity employee, the employer is obliged to send such a person (employee) to a preliminary (periodic) medical checkup.

3. The procedure for conducting mandatory preliminary and periodic medical examinations of workers engaged in the activities specified in the resolution is determined by order of the Ministry of Health and Social Development of Russia dated April 12, 2011 N 302n "On approval of lists of harmful and (or) dangerous production factors and work, during the performance of which mandatory preliminary and periodic medical examinations (examinations) are carried out, and the Procedure for conducting mandatory preliminary and periodic medical examinations (examinations) of workers engaged in hard work and work with harmful and (or) dangerous working conditions "(registered Ministry of Justice of the Russian Federation on October 21, 2011, registration N 22111).

In accordance with federal law dated March 30, 1999 No. 52-FZ "On the sanitary and epidemiological well-being of the population" and in pursuance of the Order of the Ministry of Health and social development of the Russian Federation dated April 12, 2011 No. 302n "On approval of the lists of harmful and (or) hazardous production factors and work, during the performance of which preliminary and periodic medical examinations (examinations) are carried out, and the procedure for conducting mandatory preliminary and periodic medical examinations (examinations) of workers employed in hard work and in work with harmful and (or) dangerous working conditions "(hereinafter - Order No. 302n) we order:

1. Approve:

1) guidelines on conducting mandatory preliminary and periodic medical examinations (examinations) of workers engaged in hard work and work with harmful and (or) dangerous working conditions (hereinafter referred to as PMO) (Appendix No. 1);

2) the form of the list of contingents subject to preliminary and periodic medical examinations (Appendix No. 2);

3) shape name list persons subject to periodic medical examinations in a medical institution (occupational pathology center) (Appendix No. 3);

4) methodological recommendations on the organization of mandatory pre-employment and periodic medical examinations of employees in order to prevent the spread infectious diseases(declared contingents) (Appendix No. 4);

5) the range of studies, the frequency of conducting preliminary and periodic preventive medical examinations of employees in order to prevent the spread of infectious diseases (declared contingents) (Appendix No. 5).

2. Managers public institutions healthcare of the Sverdlovsk region:

1) bring the material and technical base and personnel of the medical organization in line with the requirements of Order No. 302n;

2) organize work on conducting PMO in accordance with Annexes No. 1 - 5 of this Order;

3) ensure the execution of documents during the PMO in accordance with the forms approved by this Order;

4) quarterly provide a report on the conduct of PMO to the FBSI "Ekaterinburg Medical - Scientific Center for the Prevention and Health of Industrial Workers" of Rospotrebnadzor (hereinafter - the regional center of occupational pathology) in accordance with Appendix No. 1.6 by the 10th day of the month following the reporting quarter;

5) to send medical specialists for short-term training cycles on the organization and conduct of PME;

6) to ensure the conduct of screening examinations for the purpose of early detection of oncological diseases of visible localizations in patients referred for preliminary and periodic medical examinations in examination rooms of institutions.

3. Head of the Health Department of the Administration of Yekaterinburg A.A. Dornbush organize:

1) bringing the material and technical base in line with the requirements of Order No. 302n and personnel medical institutions in Yekaterinburg;

2) work on conducting PMO in accordance with appendices No. 1 - 5 of this Order;

3) execution of documents during the PMO in accordance with the forms approved by this Order;

4) quarterly provision of a consolidated report on medical institutions in Yekaterinburg on conducting PMO to the regional center of occupational pathology before the 10th day of the month following the reporting quarter;

5) direction of medical specialists for short-term training cycles on the organization and conduct of PME;

6) conducting screening examinations for the purpose of early detection of oncological diseases of visible localization in patients referred for preliminary and periodic medical examinations in examination rooms of institutions.

4. 5. Head of Licensing and Quality Control Department medical care Ministry of Health of the Sverdlovsk Region Kharlamova N.A., Chief Freelance Occupational Pathologist of the Ministry of Health of the Sverdlovsk Region Rosla N.A. carry out control and supervision measures in relation to medical organizations of all forms of ownership engaged in preliminary and periodic medical examinations in the Sverdlovsk region in accordance with Federal law.

6. For bosses territorial departments Department of Rospotrebnadzor in the Sverdlovsk region, the chief physicians of the FBUZ "Center for Hygiene and Epidemiology in the Sverdlovsk Region" to strengthen control over the conduct of mandatory preliminary and periodic medical examinations (surveys) of workers employed in heavy work and in work with harmful and (or) dangerous working conditions " , to carry out control and supervisory activities in accordance with the Federal legislation.

7. To the head of the department of specialized medical care, incl. of high-tech assistance of the Ministry of Health of the Sverdlovsk Region Chadova E.A., Head of the Occupational Health Supervision Department Ruzakov V.O., together with the chief freelance occupational pathologist of the Ministry of Health of the Sverdlovsk Region Rosla N.A.:

1) organize training seminars on the implementation of this Order in medical institutions, medical organizations and bodies of Rospotrebnadzor in the Sverdlovsk region;

2) by June 1, 2012, develop programs for short-term additional vocational education for medical specialists who have not previously been trained.

WE ORDER:

1. Approve:

2. Heads of state healthcare institutions of the Sverdlovsk region:

1) bring the material and technical base and personnel of the medical organization in line with the requirements of Order No. 302n;

2) organize work on conducting PMO in accordance with annexes No. 1-5 of this order;

3) ensure the execution of documents during the PMO in accordance with the forms approved by this order;

    1. 7. To the head of the department of specialized medical care, incl. of high-tech assistance of the Ministry of Health of the Sverdlovsk Region Chadova E.A., Head of the Occupational Health Supervision Department Ruzakov V.O., together with the chief freelance occupational pathologist of the Ministry of Health of the Sverdlovsk Region Rosla N.A.:

    2. 1) organize training seminars on the implementation of this order in medical institutions, medical organizations and bodies of Rospotrebnadzor in the Sverdlovsk region;

    3. 2) by 01.06.2012, develop programs for short-term additional professional education for medical specialists who have not previously received training.
8. To recommend the regional center of occupational pathology to provide organizational, methodological and advisory assistance in organizing and conducting PMO, medical institutions conducting PMO.

    1. 9. Recommend the heads of enterprises (organizations) to organize work on the organization and conduct of PMO in accordance with this order.

    2. 10. To impose control over the execution of this order on the Deputy Minister of Health of the Sverdlovsk Region Turkov S.B. and Deputy Head of the Office of Rospotrebnadzor for the Sverdlovsk Region Vlasov I.A.

Guidelines on conducting mandatory preliminary and periodic medical examinations (examinations) of employees engaged in hard work and work with harmful and (or) dangerous working conditions at enterprises and organizations of the Sverdlovsk region

Preliminary and periodic medical examinations are carried out by medical organizations that have the right to conduct preliminary and periodic medical examinations, as well as to examine professional suitability in accordance with the current regulatory legal acts.


  1. The procedure for conducting preliminary medical examinations
To pass the examination, a person entering a job submits to medical organization the following documents:

To ensure the quality of the examination, the medical commission of the medical organization evaluates the data presented in the referral (Appendix No. 1.1 to this order) and the health passport (Appendix No. 1.2 to this order). The medical commission independently evaluates documents using the data of the Rospotrebnadzor Administration for the Sverdlovsk Region, which are posted on the website ( www.ocsen.ru), this information can also be obtained upon request at the Territorial Department of the Office of Rospotrebnadzor for the Sverdlovsk Region, or with the involvement expert organizations or experts.

Based on the results of the assessment of documents, the medical commission of the medical organization determines the scope of the examination in accordance with the Procedure approved by Order No. 302n.

For a person undergoing a preliminary examination, a medical organization issues:

Outpatient medical record ( accounting form N 025 / y-04, approved by the Order of the Ministry of Health and Social Development of Russia dated November 22, 2004 N 255) (registered by the Ministry of Justice of Russia on December 14, 2004 N 6188) (hereinafter referred to as the medical record), which reflects the conclusions of medical specialists, the results of laboratory and instrumental studies, conclusion based on the results of a preliminary medical examination;

The employee's health passport, with completed items 11-14 (Appendix No. 1.2 to this order).

The preliminary examination is completed in the case of an examination of the person entering the work by all medical specialists, as well as the completion of the full scope of laboratory and functional studies provided for by the List of harmful and (or) hazardous production factors, in the presence of which mandatory preliminary and periodic medical examinations are carried out ( examination) factors (Appendix 1 to Order No. 302n) (hereinafter referred to as the List of factors) and the List of works, during the performance of which mandatory preliminary and periodic medical examinations (examinations) of employees are carried out (Appendix 2 to Order 302n) (hereinafter referred to as the List of Works).

Upon completion of the preliminary examination by the person entering the workforce, the medical organization that conducted the PMO, a conclusion is drawn up based on the results of the preliminary (periodic) medical examination (hereinafter referred to as the Conclusion) (Appendix No. 1.3 to this order).

The conclusion is drawn up in two copies, one of which is issued to the person entering the work, on hand against signature, and the second is attached to the medical card of the outpatient.


  1. The procedure for conducting periodic medical examinations

The organization of periodic medical examinations (hereinafter referred to as PMO) is carried out by the employer, which ensures compliance with the timeliness and frequency of PMO.

The frequency (frequency) of conducting PMO, the volume of laboratory and instrumental studies is determined in accordance with Order No. 302n, can be increased if necessary on the basis of proposals from the bodies exercising state sanitary and epidemiological supervision, decisions of state authorities of the constituent entities of the Russian Federation or bodies local government in individual organizations, due to the unfavorable sanitary and epidemiological situation.

The employer submits an application to a medical organization that has an appropriate license to conduct PMO, in which it indicates the number of persons subject to PMO in accordance with the list of contingents, and the desired timing of the PMO. Based on the results of consideration of the application, the medical organization sends an offer to the employer indicating the start date of the PMO.

PMOs are carried out on the basis of an agreement concluded with a medical organization and a list of names of persons subject to periodic medical examinations (Appendix No. 3 to this order), developed on the basis of a list of contingents of workers subject to periodic and (or) preliminary examinations (hereinafter referred to as lists of names) indicating harmful (dangerous) production factors, as well as the type of work in accordance with Order No. 302n (Appendix No. 2 to this order).

The following employees are subject to inclusion in the contingent lists and name lists:

Exposed to harmful production factors specified in the approved List of factors, as well as harmful production factors, the presence of which was established based on the results of attestation of workplaces for working conditions, carried out in the prescribed manner. As a source of information about the presence of harmful production factors at workplaces, in addition to the results of attestation of workplaces for working conditions, the results of laboratory research and testing obtained as part of control and supervision activities, production laboratory control, as well as operational, technological and other documentation for machines, mechanisms, equipment, raw materials and materials used by the employer in the implementation of production activities;

Performing the work provided for by the approved List of Works.

The above lists (Appendix No. 2, No. 3 to this order), developed and approved by the employer, are sent within 10 days to the Territorial Departments of the Office of the Federal Service for Supervision of Consumer Rights Protection and Human Welfare in the Sverdlovsk Region (hereinafter referred to as the Office of Rospotrebnadzor for Sverdlovsk region). The Department of Rospotrebnadzor for the Sverdlovsk Region evaluates the validity, completeness and reliability of the information included in the lists of contingents and name lists. Information on the compliance (non-compliance) of the lists of contingents and lists of names submitted for consideration with the requirements of sanitary legislation is posted on the official website of the Office of Rospotrebnadzor for the Sverdlovsk Region ( www.ossen.ru) indicating the full name of the organization that submitted the documents, the name of the person who approved these documents and the date of approval, indicating the reasons for the non-compliance of the documents.

Primary documents(contingents and lists of names), necessary for organizing and conducting periodic medical examinations of persons in the Centers of Occupational Pathology (participants emergencies or incidents, workers employed in work with harmful and (or) hazardous substances and production factors with a single or multiple excess of the maximum allowable concentration (MPC) or maximum allowable level (MPL) for the current factor, workers whose workplaces do not have information about concentrations, levels, parameters of factors of the working environment and the labor process 1 year or more, employees who have (had) a conclusion on a preliminary diagnosis of an occupational disease, persons from a group of increased risk of developing occupational diseases, persons with persistent consequences of accidents at work, as well as others employees in the event of a decision by the medical commission) is subject to examination in institutions (their territorial divisions, branches) accredited in the prescribed manner to conduct sanitary and epidemiological examinations (orders of the Rospotrebnadzor of the Russian Federation dated November 12, 2007 No. 319 “On the list of organizations carrying out sanitary and epidemiological examinations” (registered by the Ministry of Justice of Russia on 03.12.2007, registration No. 10603) and dated 24.01.2008 No. 14 “On making additions to the order of Rospotrebnadzor dated 12.11.2007 No. 319”, registered Ministry of Justice of Russia on February 4, 2008, registration number 11098). Examination of documents is carried out within 1 month after payment for expert work. Based on the results of the examination, a report is drawn up, which is submitted to the bodies of Rospotrebnadzor, and an entry is made in the name lists about the results of the examination. If the results of the examination are positive, the name lists are sent to the Occupational Pathology Center for organizing and conducting medical examinations, and if the results are negative, they are sent for revision along with the report.

Not later than 2 months before the start date of PMO agreed with the medical organization, the employer sends a list of names to the specified medical organization for drawing up a contract for PMO.

Officials responsible for conducting PMO prepare referrals for each employee in the recommended form (Appendix No. 2 to this order) and hand them over to employees against signature.

The medical organization, within 10 days from the date of receipt of the list of names from the employer (but no later than 14 days before the start date of the PMO agreed with the employer), on the basis of the specified list of names, draws up a calendar plan (hereinafter referred to as the calendar plan).

The calendar plan is coordinated by the medical organization with the employer (his representative) and approved by the head of the medical organization.

The employer, no later than 10 days before the start date of the PMO agreed with the medical organization, is obliged to familiarize the employees subject to periodic examination with calendar plan.

The medical commission of a medical organization, on the basis of the hazardous production factors or work specified in the list of names, determines the need for participation in the PMO of the relevant specialist doctors, as well as the types and volumes of necessary laboratory and functional studies.

To undergo a periodic examination, the employee must arrive at the medical organization on the day established by the calendar plan and present the following documents to the medical organization:

Referral for a preventive medical examination (Appendix No. 1.1 to this order);

Passport (or other document standard pattern certifying his identity);

The employee's health passport, with completed points 1-10 (Appendix No. 1.2. to this order);

The decision of the medical commission that conducted the mandatory psychiatric examination(in cases stipulated by the legislation of the Russian Federation).

For an employee undergoing a periodic examination, the following documents are drawn up in a medical organization:

Medical record of an outpatient (registration form N 025 / y-04, approved by Order of the Ministry of Health and Social Development of Russia dated November 22, 2004 N 255) (registered by the Ministry of Justice of Russia on December 14, 2004 N 6188) (hereinafter referred to as the medical card), which reflects the conclusions medical specialists, the results of laboratory and instrumental studies, conclusions based on the results of a preliminary or periodic medical examination;

The employee's health passport, with completed items 11-14 (Appendix No. 1.2 to this order).

Upon completion of the PMO, the medical organization draws up the Conclusion of the preliminary (periodic) medical examination (Appendix No. 1.3 to this order). The conclusion is drawn up in two copies, one of which, based on the results of the medical examination after the completion of the examination, is issued to the employer, and the second is attached to the medical record of the outpatient.

Based on the results of the periodic inspection, the employee’s belonging to one of the dispensary groups is determined, in accordance with the current regulatory legal acts, with subsequent registration in the medical card and health passport recommendations for the prevention of diseases, including occupational diseases, and if there are medical indications - for further observation, treatment and rehabilitation.

A medical organization with which the employer has not renewed the contract for conducting preliminary and (or) periodic examinations of employees, at the written request of the employer, must transfer, according to the inventory, the medical records of employees to the medical organization with which the employer has currently entered into an appropriate contract.

The occupational pathology center with which the employer has not renewed the contract for preliminary and (or) periodic examinations of employees, at the employer’s written request, must transfer the employees’ medical records according to the inventory to the occupational pathology center with which the employer has currently entered into an appropriate agreement.

Participants in emergencies or incidents, workers employed in work with harmful and (or) hazardous substances and production factors with a single or multiple excess of the maximum allowable concentration (MAC) or maximum allowable level (MPL) for the current factor, workers at whose workplaces there is no information on concentrations, levels, parameters of factors of the working environment and the labor process for 1 year or more, employees who have (had) a conclusion on a preliminary diagnosis of an occupational disease, persons from a group of increased risk of developing occupational diseases, persons with persistent consequences of industrial accidents, as well as other employees, if the relevant decision is made by the medical commission, at least once every five years, undergo periodic inspections in the centers of occupational pathology (Regional Center for Occupational Pathology, Center for Occupational Pathology at the Sverdlovsk Regional Clinical Hospital No. 1, City Center for Occupational Pathology MAU "City Clinical Hospital No. 40"), depending on the area of ​​supervision of the center (order of the Minister of Health of the Sverdlovsk Region and the Chief State Sanitary Doctor in the Sverdlovsk region No. 434-P / 01 / 1-176 of June 21, 2003 “On measures to improve the organization of occupational pathological care in the Sverdlovsk region - hereinafter Order No. 434-P / 01 / 1-176).

In the event that a psychiatrist and narcologist identify persons with suspected medical contraindications corresponding to the profile of these specialists, to be admitted to work with harmful and (or) dangerous production factors, as well as to work, during which preliminary and periodic medical examinations are mandatory (examinations) of employees, these persons, in cases provided for by the legislation of the Russian Federation, are sent for examination by a medical commission authorized by the Ministry of Health of the Sverdlovsk Region.

In case of suspicion that an employee has an occupational disease during a periodic examination, the medical organization draws up and sends, in the prescribed manner, a notice of the establishment of a preliminary diagnosis of an occupational disease to the Rospotrebnadzor body in the Sverdlovsk Region and issues a referral to the employee to the occupational pathology center in accordance with his area of ​​supervision for additional examination ( Order No. 434-P / 01 / 1-176).

In cases of difficulty in determining the professional suitability of an employee due to his illness and for the purpose of examining professional suitability, the medical organization sends the employee to the regional center of occupational pathology for additional examination in order to complete the examination of professional suitability.

Based on the results of the examinations, the medical organization, no later than 30 days after the completion of the PMO, together with the employer and the Rospotrebnadzor Administration for the Sverdlovsk Region, summarizes the results of the periodic medical examinations of employees and approves the final act (Appendix No. 1.4. to this order). The final act is drawn up in four copies and sent within 5 days from the date of approval to the employer, the Office of Rospotrebnadzor for the Sverdlovsk Region and the Regional Center for Occupational Pathology. One copy of the final act is stored in the medical organization that conducted periodic examinations for 50 years.

Based on the results of periodic medical examinations, medical institutions quarterly submit a report to the regional center of occupational pathology by the 10th day of the month following the reporting quarter with final acts in electronic form.

The Regional Center for Occupational Pathology of the Sverdlovsk Region summarizes and analyzes the results of periodic medical examinations of workers employed in jobs with harmful and (or) dangerous working conditions in the territory of this constituent entity of the Russian Federation and no later than February 15 of the year following the reporting one sends summarized information to the Federal Center for Occupational Pathology Ministry of Health and Social Development of Russia and to the health management body of this constituent entity of the Russian Federation.
